Responsibilities:

• Provide primary care to patients while receiving rehabilitative care in our specialty hospitals
• Collaborate with treatment team members for medication management
• Implement an individualized plan of care for patients' goals
• Complete Medicare required visits during hospital stay
• Complete acute, routine follow up, and discharge visits, and transitional care
• Make recommendations on DME, home health services, or outpatient services post discharge
